What is arthroscopic shoulder surgery and how is it performed?
Arthroscopic shoulder surgeries are performed on an operating table specifically designed for this purpose. Generally, with the possibilities provided by this table, the patient is operated on in a sitting position. Other setups allow the patient to be operated on while lying on their side. A camera is inserted into the joint through an approximately 1 cm incision. Then, using surgical instruments inserted through a few similar-sized incisions, the surgery is performed. What are the advantages and disadvantages of arthroscopic shoulder surgeries?
Advantages of arthroscopic shoulder surgeries are as follows:
1. Small and cosmetically acceptable scars.
2. Can be performed at any age.
3. No need for blood transfusion.
4. Easy access to areas in the joint that are difficult to reach with open techniques.
5. Multiple procedures that would require several incisions can be performed through small incisions with close-up visualization.
6. Due to smaller incisions, surrounding tissues heal faster compared to open surgery methods. Consequently, the time to start physical therapy after surgery is reduced.
Disadvantages of arthroscopic shoulder surgeries are:
1. Cannot be used for all shoulder-related problems (e.g., vascular, nerve repair, or masses related to these tissues around the shoulder).
2. Cannot be used for problems such as cysts or masses within the bone.
3. The shoulder is close to both the neck root and the armpit, surrounded by many blood vessels and nerves. Although rare, there is a risk of injury to these structures during surgery. This risk is minimized by opening incisions using specific reference points and inserting plastic tubes called “cannulas” into the incision sites. Cutting and piercing surgical instruments are then used through these tubes to prevent vascular and nerve injuries.
